A 45-year-old woman of South Asian origin presents with sudden weakness of her left arm, weakness of her right leg, and left-sided neglect for 36 hours. There was no associated slurring of speech, difficulty in swallowing, or sensory disturbances. She is right-handed.
Her medical history is significant for a previous miscarriage at 16 weeks. There is no family history of stroke, heart disease, or thrombotic disorders. She is not on oral contraceptives and does not smoke or consume alcohol. She denies using recreational drugs.
Serial ECGs are found to be normal. Her random capillary blood glucose is 105 mg/dL. Her full blood count, ESR, CRP, serum electrolytes, and liver and renal profiles are all normal. Non-contrast CT brain is unremarkable.
